Cancellation form

You only need this if you want to use your statutory 14-day right to cancel. You do not have to use this form: an email to info@kolvishop.com saying you want to cancel is just as valid. This is the model form set out in the Consumer Contracts (Information, Cancellation and Additional Charges) Regulations 2013, reproduced here so it is available to you.

What happens next

• You have 14 days from the day you, or someone you nominated, take delivery. If your order came in more than one parcel, the 14 days start on the day the last parcel arrived.
• Send the goods back within 14 days of telling us. We pay the return postage.
• We refund within 14 days of getting the goods back, or of you showing us they have been sent, whichever comes first. The refund goes back to the card or account you paid with, and it includes the standard delivery charge you paid on the way out.
• The right to cancel does not apply to anything sealed for hygiene reasons, which includes all of the supplements, once the seal is broken. Our 60-day guarantee covers those instead.
• A refund can be reduced if goods have been handled more than you would handle them in a shop.

Your other rights are unaffected

Separately from all of this, everything we sell has to be of satisfactory quality, fit for purpose and as described. If it is not, the Consumer Rights Act 2015 gives you a full refund if you tell us within 30 days of delivery, and a repair, replacement or refund after that. Nothing on this page cuts those rights down.
